Appointments
Setting up an appointment is as simple as calling the EAP office. You will be offered an appointment time, generally within five working days. Clients with emergency situations may receive same-day attention.
In emergency or crisis situations, an EAP counselor is available by phone 24 hours a day, 7 days a week at 800-832-7733.
Office Locations
Office locations are found throughout the state including: Bountiful, Ogden, Salt Lake City, Murray, Sandy, West Valley, Provo, Cedar City, and St. George. Intermountain EAP also contracts with therapists in Fredonia and Kanab. An appointment may be made at any of these locations by calling the number listed above.
Confidentiality
The EAP follows federal (HIPAA) and state laws that require strict confidentiality of client records. The law mandates that in cases of child abuse, elder abuse, or where a person may be a threat to their or someone else’s safety, the counselor is a mandated reporter and must notify the proper authorities.
